Intent to Complete a Campus Certificate

UIC offers two types of certificates, IBHE certificates and campus certificates.
If you intend to file for completion of a campus certificate that you are taking, you must formally apply to/add the certificate to your curriculum/student record AND complete an intent of completion.
For all programs under the Graduate College, you must complete the Change of Graduate Program or Degree-sought for Current Graduate Students (i.e. Request to Change or Add a Graduate Program) form. Please route the form to your current/primary program, the certificate to which you are applying/adding, OIS (for F-1 and J-1 students) and finally to the Graduate College for final approval.
