International students who are residing outside of the United States while taking courses at the University of Illinois should submit a waiver with their current insurance information at this link: https://campuscare.uic.edu/.
For help completing the waiver, please go to https://campuscare.uic.edu/waivers-reinstatement-forms/.
If you are unable to complete the waiver form due lack of insurance information or any other reason, please contact the department of CampusCare at cchealth@uic.edu in order for assistance.
