The forms in this section are for use by applicants/students for the various awards listed.  Faculty mentor forms, where relevant, are also listed.  Transmittal and rating forms used by the department are not listed here.

  • Application for Graduate Appointment (NO LONGER REQUIRED for Graduate College fellowships and awards)
  • Letter of Recommendation Cover Form (for all of the Graduate College fellowships -- not required for awards, e.g., Award for Graduate Research, Provost's Graduate Research Award, Provost's Graduate Internship, or the Outstanding Thesis/Dissertation Award)

The forms available through the link below are to be used by Directors of Graduate Studies and departmental/program staff (eg. transmittal forms).  These forms are Bluestem protected and can only be downloaded by those individuals for whom the Graduate College has provided access.
