FMC Educational Fund Fellowship

2010-2011 Application Instructions

Description: Three to five awards of approximately $9000 each, plus a tuition and selected fees waiver, for one academic year. Awards are not renewable, but previous winners are welcome to reapply.

Application Deadline: UIC on-campus deadline is 4:00 p.m., Friday, January 22nd, 2010.

Eligibility: UIC full-time graduate students in business, engineering, or related areas, who are not graduating in Spring 2010 (i.e., who will continue their studies full-time in 2010-2011). Preference is given to Master’s candidates in business administration and engineering, although doctoral students may also apply. There is no citizenship requirement.

Application: The following materials must be submitted by the campus deadline:

    1. FMC Educational Fund Fellowship Transmittal Form: This is a fillable PDF form which the Director of Graduate Studies or Department Head should complete, print out, sign, and include with your application.

    2. Notice of Graduate Admission: this is the letter from the UIC Graduate College confirming your admission to a graduate program. If you have not retained a copy, your department should have one on file.

    3. Application for Graduate Appointment: completed and signed by the applicant. Click on the link to obtain this form.
IMPORTANT!  In lieu of the 300-word statement, you must include a Background Essay, as described below. Do not write “refer to résumé” and include a résumé; the résumé will NOT be reviewed by the selection committee.

    4. Three Letters of Recommendation: click on the link to obtain this form.  Recommendation letters must be written specifically for your application to the FMC award -- letters from your application to graduate school are not permitted.  Recommendation letters should come in sealed envelopes, as part of the entire packet.

    5. Background Essay
: this is a two- to three-page narrative describing you, your graduate achievements, and what you want to do once you have completed the degree. Essays should be double-spaced in Times New Roman 12-point font with 1” margins all around.

    6. Transcripts: one official UIC transcript.  If this is your first semester at UIC, certified copies of the post-secondary transcripts that you submitted with your application to graduate study are acceptable.

PLEASE NOTE:

  • Send the application in one packet; do NOT send parts of the application separately as they are completed.
  • Do not use staples.
  • Please do not send materials other than those described above, such as résumés, vitas, published articles, TOEFL scores, etc. They will not be reviewed by the selection committee.
  • Please do not put your parts of the application (i.e., notice of admission, application form and essay) in separate envelopes, attach your own cover sheets or letters, or provide tabs to identify parts of the application.  This is an unnecessary waste of paper.
  • Incomplete applications will not be accepted.

Applications turned in after 4:00 p.m. Friday January 22nd will not be accepted.

The complete application must be submitted to:
